ANSWER

Employee must:

  • Review the Disability Checklist prior to medical leave of absence.
  • Supervisors are to be notified about the length of time the employee expects to be disabled and the employee must request approval for a leave of absence.
  • Disability claims should be reported online at www.mylibertyconnection.com with the company code “University” OR if the employee does not have computer access, he/she must complete the Liberty Mutual Disability Claim Forms.
  • If the employee’s hire date is less than 18 months from his/her date of disability, he/she must file for State Disability Insurance (SDI) in addition to the Liberty Mutual claim.

Supervisor must:

  • Work with the employee before the start of the leave to complete the Department Disability Claim Statement online or by contacting a Disability Management Consultant by coordinating point.
  • If the employee becomes eligible for Partial Disability: Stay at Work/Return to Work program, contact Wendy Pawling and the DM Consultant.  If the employee becomes eligible for the Transitional Work Program refer to the Transitional Work Policy and Program Guidelines and contact the Disability and Rehabilitation Consultant.

Catastrophic Leave - permits salary and benefits continuation for non-represented and eligible represented UCI staff who have exhausted all paid leave due to their own serious illness or injury or to care for a seriously ill member of the employee's family or household.  Salary and benefits continuation is achieved through donation of vacation credits from other University employees.
